Drum to disc conversion help - Please!

Can somebody give me a link or something to a drum to disc brake conversion? Also it'd be helpful if Faction_03 could repost a bigger picture of his parts list.

Got new rims and boy do drums look stupid with em. Found a f/r slot/cross drill rotor set for $200 but he doesn't sell em as just front...
